Description
Mavorixafor (sold under the brand name Xolremdi) is an oral CXCR4 receptor antagonist. It is the first FDA-approved therapy specifically indicated to increase circulating white blood cell counts (mature neutrophils and lymphocytes) in patients aged 12 and older with WHIM syndrome.
Mechanism of Action
In WHIM syndrome (Warts, Hypogammaglobulinemia, Infections, and Myelokathexis), a genetic mutation causes overactivation of the CXCR4 receptor. This traps mature white blood cells in the bone marrow and prevents them from entering the bloodstream. Mavorixafor blocks this receptor, preventing it from binding to its ligand (SDF-1α). This mobilization releases immune cells into circulation, bolstering the body’s ability to fight infections.
Key Clinical Benefits
- Fewer Infections: Clinical trials have shown that patients taking mavorixafor experience significantly lower annualized infection rates.
- Wart Reduction: Studies indicate a marked, gradual disappearance of refractory warts in patients undergoing treatment.
- White Blood Cell Mobilization: Leads to a rapid and sustained increase in the time-above-threshold (TAT) for absolute neutrophil and lymphocyte counts.
- Formulation: Oral capsules taken once daily.
- Dosage Adjustments: The standard dose may need to be reduced in patients taking certain moderate or strong CYP3A4 inhibitors (e.g., to 200 mg/day) and should generally be avoided if the patient is taking strong CYP3A4 inducers.
- Age Limit: Established for patients 12 years and older.
- Common Side Effects: The most frequent adverse reactions (>10% of patients) include thrombocytopenia, pityriasis, rash, rhinitis, epistaxis, vomiting, and dizziness.
- Pregnancy & Nursing: Breastfeeding is not recommended during treatment and for three weeks following the final dose.
- Organ Impairment: It is not recommended for patients suffering from severe renal impairment or moderate to severe hepatic impairment.
