Description

Methylene blue (methylthioninium chloride) is a synthetic salt and vibrant dye initially developed in 1876. Its only FDA-approved medical use is to treat methemoglobinemia, a life-threatening blood condition. While biohackers promote it for anti-aging and memory, these off-label uses carry significant, life-threatening risks. 

Approved Medical Uses

  • Methemoglobinemia: Administered intravenously by a physician, it helps convert oxidized iron in red blood cells back to its functional, oxygen-binding state.
  • Diagnostics & Dyes: Used by surgeons to identify and delineate certain tissues and tumors during operations.
  • History: It was one of the first fully synthetic drugs used to treat malaria and various infections over a century ago. 
Biohacking and Wellness Claims
Methylene blue is increasingly marketed in unregulated internet supplements as a cognitive enhancer, mitochondrial booster, and anti-aging agent. Proponents claim it aids in treating brain fog, dementia, and Alzheimer’s disease. However, the Harvard Health medical advisory board warns that rigorous clinical trials in humans supporting these daily supplement claims are lacking. 

Risks and Side Effects

Unlike regulated pharmaceuticals, over-the-counter methylene blue supplements are often unstandardized and may contain toxic contaminants. Important clinical risks include:
  • Serotonin Syndrome: Because methylene blue is a mild monoamine oxidase inhibitor, combining it with antidepressants or pain medications can cause a life-threatening surge of serotonin in the brain. 
  • Hypertensive Crises: Consuming unregulated methylene blue alongside tyramine-rich foods (like aged cheeses, red wine, or cured meats) can trigger dangerously high blood pressure. 
  • Tissue Damage: Injecting methylene blue near nerves improperly can cause permanent nerve death. 
  • Red Blood Cell Toxicity: Excessive doses or use in individuals with certain enzyme deficiencies can destroy red blood cells. 
  • Physical Staining: It is a potent textile dye that permanently stains skin, nails, clothing, and carpets blue.
